Payment info for all the spring/summer activities:

Virgils Summer Skills Clinics:  $200 each, check written to Virgil Hooe and mailed to 9650 Birch Road  Anchorage, AK  99507

Elite Team Training: $125, check written to Virgil Hooe and mailed to 9650 Birch Road  Anchorage, AK  99507

All – American Elite Camp: Level 1 & 2 are $250, Level 3 is $245.  Deposit of $75 to be mailed in with the application, the remainder due the first day of camp or you can pay in full with application.  Checks written to MSVB and mailed to Dana Aasland 3400 Eastwind Dr.  Anchorage,  AK 99516

Festival Payments: refer to the Festival news page for plan.  Checks written to MSVB and mailed to MSVB Accounting @ 6930 Viburnum Anchorage, AK  99507

Cost for the 2012-13 season
will be $725

a deposit of $225 is due when you turn in your player packet (financial contract & USAV medical form, found on our FORMS page)  at your 2nd evaluations

Payment options and due dates:

one lump sum: (INCLUDING the initial $225) of $725.00 at the 2nd evaluation.

1 payment: The payment (after the initial $225) of $500.00 is due by January 1st, 2013

2 payments:The first payment (after the initial $225) of $250.00 is due by January 1st, 2013.
The second payment of $250.00 is due by March 1st, 2013

4 payments: The first payment (after the initial $225) of $125.00 is due by January 1st, 2013.  The 3 payments of $125.00 is due by the 1st of each of the following months: February, March, April.

If your checks have a  last name that is different then the last name of the player please put the players name in the memo portion of the check so payment can be properly applied to the right player.
